First Visit
Your First Visit is All About You:

Welcome and we are very excited and honored to have you as a member of our dental family! At your initial appointment, our office standard of care will consist of a complete set of digital x-rays and intraoral photos, a comprehensive exam, soft tissue exam, and consultation regarding your treatment options. If your treatment plan requires a more extensive procedure, we will schedule another appointment for treatment or refer you to the necessary specialist. If you are in extreme pain, we will do our best to see you as soon as possible. We will make every effort to relieve your pain through emergency treatment, and then schedule you for the necessary follow-ups.
Bring your smile to your first appointment and these additional items:
- Your referral slip and any X-rays, if applicable
- A list of medications you are presently taking
If you have dental insurance, please let us know ahead of time so we can obtain your dental benefits prior to the appointment. At the time of the appointment, please bring your current insurance card. If you don’t have one, we will need the subscriber number, subscriber birth date, patient birth date, group number, and phone number. This will save time and allow us to help you in the processing of your claim.
PLEASE NOTE: All patients under the age of 18 years must be accompanied by a parent or guardian during the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (e.g. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on ANY medication (e.g. heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist or physician has taken X-rays, please have them forwarded to our office as they may be helpful in our diagnosis. A digital radiograph can be emailed to us at [email protected]. If additional films are necessary, they can be taken at our facility.
